Printable Face Template

I am currently customizing my website which i am using as a template. It appears you already have fillable fields created from your screen shot. This is also where i got this template from. I need to be able to create a fillable pdf where certain information can be merged from an excel spreadsheet into the fillable pdf so our land use specialists can complete while in the field using tablets. Not sure why it updated without an actual update, but it works now.

I also need to be able to create a field where the user can attach or import a jpg. Building a puppet from scratch. I've looked at the character animator tutorial titled: For example, the common number for sharpest resolution is 300 ppi, but most seem to only go as high as 200 ppi in order to reduce file.
